What to Bring to Your Appointment
If you are a first-time patient, please plan on arriving early to complete a patient registration form. (You can download the forms and fill them before) Please bring the following items to the appointment:

• Insurance card(s)
• Referral forms (if required)
• List of all prescription and over-the-counter medications that you are currently taking
• Any required co-payment
• Pertinent medical and surgical history information
• X-ray films that you may have

Appointments
Please call the office to make an appointment in advance or request an appointment on line. If you're unable to keep your appointment, please call us as far inadvance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we're are late due to emergencies or hospital surgery.

Insurance and Payment Information
We encourage all patients to confirm their coverage prior to making an appointment. We accept most major Insurance plans. Co-payments are due at the time of the appointment. You are responsible for any unpaid balance not covered by insurance. Please call us with any questions regarding which insurance plans we accept or other billing concerns. Insurance coverage and benefit questions should be directed to your health insurance provider.

Prescriptions & Renewals
Please request all prescriptions and authorizations for renewals between 8:30am - 4:00pm,Monday to Friday, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances.
